Achievement: met over set, as the form's first number
Four of five objectives met is 80%, and that figure is the spine of the self-appraisal: every comment hangs off an objective, and the count says how many comments are about delivery and how many about a miss. The worksheet returns the figure so the form opens with evidence rather than an adjective.
Exceeded, and where you placed yourself on the scale
Two of the four met objectives exceeded their measure is 50%, which is the case for a rating above the middle of the scale; a self-rating of 4 out of 5 is 80% of the scale, and the manager's rating will be read against both. The scale is your company's, the rating is yours, and the worksheet publishes no rule about where they should meet.
Carried forward, and the comment that goes with it
Set less met is the objective that carries into the next period, one on the worked example, and the comment for a missed objective is the one that matters most: what changed, what was agreed and what is live now. What should I write in self-appraisal comments?
One comment per objective, each with what was set, what was delivered, when, and a figure: a date against a target date, a count against a target count, a saving against last year. The examples above this worksheet are the shape; the worksheet gives you the achievement and exceeded figures to open with.
How do I rate myself in a self-appraisal?
Against your company's scale and your own evidence: the achievement figure and the exceeded share are the case for the rating, and the worksheet shows where a self-rating sits as a share of the scale. It publishes no rule about which rating the figures deserve; that is the conversation with your manager.
